HeartSong's Advanced Access policies optimize your chances of seeing your personal physician as soon as possible.

HeartSong's Advanced Communication policies facilitate your access to your physician and your personal health information outside of scheduled appointments.

In 2009, the Office of Vermont Health Access prepared a Bill of Rights Statement for beneficiaries of all Vermont Health Care Programs

Informed Consent

Informed consent refers to a process of communication between you and your physician that allows you to actively participate in your care. Informed consent gives you the right to accept or decline treatment at any time. It also gives you the right to choose a HeartSong clinician or another clinician at another medical office as your primary care provider.

Natural Pharmacy

Our natural pharmacy policies apply to non-prescription natural supplements and standardized herbal products sold through our in-house natural pharmacy.

Our prescription medication policies apply to products which you obtain at a pharmacy of your choice.

NCQA-Compliant Patient-Centered Medical Home

In 2012, HeartSong applied to the Vermont Blueprint for Health to become an Advanced Primary Care Practice (APCP) by demonstrating compliance with the standards for practice management as a Patient-Centered Medical Home established by the National Committee for Quality Assurance (NCQA) and used by the Vermont Blueprint to certify medical practices as APCP. HeartSong anticipates being fully compliant by by August 2013 and is scheduled to receive a final score from the NCQA on September. This score will be used by the VT Blueprint as the basis for certifying HeartSong as an APCP.

Privacy Practices

The Health Insurance Portability and Accountability Act (HIPAA) of 1996 (P.L.104-191) [HIPAA], enacted by the U.S. Congress in 1996, requires Secretary of Health and Human Services to issue national privacy regulations governing individually identifiable health information.
